本使用條款（下稱「本條款」）適用於歐亞盈健通訊科技有限公司(AR Wellness Communication Technology Limited，下稱「本公司」） 所提供之醫療網絡會員服務產品 「歐亞優健通(醫結會籍) 」（AR WellPass）（下稱「本服務」）。

請閣下在使用本網站、購買計劃或使用 QR Code 服務前，仔細閱讀本條款。

The following Terms & Conditions (“Terms”) apply to the medical network membership service product “歐亞優健通(醫結會籍) / AR WellPass” (“the Service”) provided by AR Wellness Communication Technology Limited (“the Company”). Please read these Terms carefully before using our website, purchasing any plans, or using any QR Code services.

1. 服務內容

1. Service Overview

1.1 「歐亞優健通(醫結會籍) 」為醫療網絡會員折扣服務，提供門診、專科、化驗等合作醫療網絡的優惠折扣。本公司並非任何形式的醫療服務提供者。

AR WellPass offers medical network discount services including GP, specialist, laboratory and other partner network benefits. The Company is not a healthcare or medical service provider.

1.2 所有醫療服務均由合作診所／醫護人員提供，本公司不參與診症、醫療意見或治療。

All medical services are provided solely by partner clinics or healthcare professionals. The Company does not provide diagnosis, medical advice, or treatment.

1.3 本公司會向會員提供 QR Code，用於於合作醫療網絡之門市核實身份及享用折扣。

The Company provides members with a QR Code, which is used for identity verification at partner medical providers and for redeeming discounts.

2. 會籍計劃

2. Membership Plans

2.1 計劃包括：

(1) 月費計劃（Monthly Plan）

(2) 年費計劃（Annual Plan）

Plans include:

(1) Monthly Plan

(2) Annual Plan

2.2 年費計劃一經購買，不設退款。

Annual plans are non-refundable once purchased.

2.3 月費計劃可於下期扣款前取消，下期付款將不會扣款，但已扣款月份不設退款。

Monthly plans may be cancelled before the next billing cycle. No further charges will be made. Fees already charged will not be refunded.

3. 資料收集及聯絡方式

3. Communication & Customer Contact

3.1 閣下同意本公司可使用電話、WhatsApp、SMS 或電郵與閣下聯絡，以提供服務支援、續期提示、會員通知或推廣資訊。

You agree that the Company may contact you via phone, WhatsApp, SMS, or email for service support, renewal reminders, member notifications, or promotional messages.

4. QR Code 使用

4. Use of QR Code

4.1 QR Code 只限會員本人使用，不可轉讓、共享或出售。

The QR Code is for the member’s personal use only and must not be transferred, shared, or sold.

4.2 合作診所／醫療服務提供者有權核實身份，如會員無法出示有效 QR Code 或身份不符，診所可拒絕提供折扣。

Partner clinics have the right to verify member identity. If a valid QR Code cannot be provided or the identity does not match, the clinic may refuse to apply any discount.

4.3 若因診所系統故障、連線問題或合作夥伴之技術問題導致 QR Code 無法使用，本公司不承擔任何責任。

The Company shall not be liable if the QR Code cannot be used due to system issues, connectivity failure, or technical problems of partner providers.

5. 合作夥伴及第三方

5. Partners & Third Parties

5.1 本服務所提供之折扣及醫療網絡由合作夥伴提供及管理，可能會因營運、政策更新或協議變動而調整。

All medical network discounts are provided and managed by partner companies and may change due to operational or policy updates.

5.2 本公司可能會向合作夥伴（包括現有及未來新增醫療網絡平台）提供必要會員資料，以供提供服務。

The Company may share essential member data with partners (including present and future medical network providers) to enable service delivery.

5.3 合作夥伴之服務質素、醫療表現、診症結果、營運時間、排期安排均不屬本公司控制。

The Company has no control over partner service quality, medical outcomes, operating hours or scheduling arrangements.

6. 免責聲明

6. Disclaimers

6.1 本公司不提供醫療意見、醫療建議或任何醫療行為。對於合作診所之醫療行為、診斷、治療結果或專業建議，本公司概不負責。

The Company does not provide medical advice or medical services. It is not responsible for any medical acts, diagnoses, treatment outcomes or professional opinions provided by partner clinics.

6.2 本公司不保證折扣之持續有效、診所之可用性、醫護人員質素或醫療安排。

The Company does not guarantee the availability or continuity of discounts, clinic participation, healthcare provider quality, or appointment arrangements.

6.3 對於因第三方（包括付款平台、IT 系統、醫療網絡平台、合作診所、WhatsApp API）造成之損失，本公司不負責。

The Company shall not be liable for any losses caused by third parties, including payment platforms, IT systems, medical network providers, partner clinics or WhatsApp API failures.

6.4 本公司不保證本網站或 QR Code 系統 24 小時無間斷運作。

The Company does not guarantee 24/7 uninterrupted operation of the website or QR Code systems.

7. 使用條款更新

7. Updates to Terms

本公司可隨時更新此條款，並於網站公佈後即時生效。

The Company may update these Terms at any time, and such changes take effect once posted on the website.

8. 適用法律

8. Governing Law

本條款受香港特別行政區法律管轄。

These Terms are governed by the laws of the Hong Kong SAR.
